State policy changes. I was drop (Nov 2016 advance letter) from my insurance therefore, I had to choose another insurer by Dec 2016, definitely not easy it was a daymare I was unable to qualify for most and choices all required some type of co-pay (Not due to AHA) ;( far less benefits, services that were free now have a co-pay, those services will no longer be used :(  I no longer have affordable dental cut from $2000 free yrly, now up to $150, I now must pay for in advance. No eyecare, all comes out of my pocket, no OTC, transportation and hearing benefits were eliminated (it's my old car or public bus) health care was free, now a co-pay. Ins letter said they will no longer pay for a needed long term med ($340) without going thru a 3 ring circus. Fortunately, I Google... Found a similar med. Several other health benefits were cut or now have a unaffordable co-pay.

Only One positive benefit FREE Silver Fit classes and gym can be used all over my city. I go to 4 gyms all very close, 20 days (January)  🏋. I'm going to set a record for me.
